Rheoliad 28
ATODLENYr wybodaeth sydd i'w rhoi i'r Cyngor gan gyflogydd athro neu athrawes gofrestredig
1. Llythyr neu hysbysiad yn terfynu cyflogaeth athro neu athrawes gofrestredig.
2. Datganiad o'r rhesymau dros y diswyddo.
3. Cofnodion y cyflogydd ynghylch y diswyddo neu unrhyw ddiswyddo a ystyriwyd, gan gynnwys nodiadau a chofnodion cyfarfodydd, nodiadau cyfweliadau, a thystiolaeth a roddwyd i'r cyflogydd neu a sicrhawyd ganddynt.
4. Cofnodion y cyflogydd ynghylch ymddygiad a arweiniodd yn y pen draw at ddiswyddo athro neu athrawes gofrestredig neu a fyddai wedi arwain y cyflogydd i ddiswyddo'r athro neu'r athrawes neu i ystyried eu diswyddo oni bai bod yr athro neu'r athrawes wedi ymddiswyddo, gan gynnwys nodiadau a chofnodion cyfarfodydd, nodiadau cyfweliadau, a thystiolaeth a roddwyd i'r cyflogydd neu a sicrhawyd ganddynt.
5. Llythyrau, rhybuddion neu hysbysiadau'r cyflogydd a roddwyd i athro neu athrawes gofrestredig mewn perthynas â diswyddo'r athro neu'r athrawes neu mewn perthynas ag ystyried eu diswyddo, neu mewn perthynas â'r ymddygiad a arweiniodd yn y pen draw at ddiswyddo'r athro neu'r athrawes neu a fyddai wedi arwain y cyflogydd i'w diswyddo neu i ystyried eu diswyddo, oni bai eu bod wedi ymddiswyddo, ac atebion neu sylwadau'r athro neu'r athrawes mewn perthynas â hwy.
6. Unrhyw ddatganiadau, sylwadau a thystiolaeth arall a gyflwynwyd gan athro neu athrawes gofrestredig i'r cyflogydd mewn perthynas â diswyddo'r athro neu'r athrawes neu mewn perthynas ag ystyried eu diswyddo, neu mewn perthynas â'r ymddygiad a arweiniodd yn y pen draw at ddiswyddo'r athro neu'r athrawes neu a fyddai wedi arwain y cyflogydd i'w diswyddo neu i ystyried eu diswyddo oni bai eu bod wedi ymddiswyddo.
7. Llythyr ymddiswyddo.
8. Unrhyw ddogfen neu wybodaeth arall sydd ym marn y cyflogydd yn berthnasol i unrhyw ymchwiliad y gallai Pwyllgor Ymchwilio ei gynnal neu unrhyw achos y gallai Bwyllgor ei ddwyn yn erbyn athro neu athrawes gofrestredig.
