Diwygiadau i Reoliadau Asiantaethau Gofal Cartref (Cymru) 2004
12.—(1) Mae Rheoliadau Asiantaethau Gofal Cartref (Cymru) 2004(1) yn cael eu diwygio fel a ganlyn.
(2) Yn Atodlen 2 (yr wybodaeth sy'n ofynnol mewn perthynas â darparwyr a rheolwyr cofrestredig asiantaeth a phersonau sydd wedi'u henwi i ddirprwyo ar gyfer person cofrestredig)
(a)yn lle paragraff 3 rhodder—
“3. Naill ai—
(a)os oes angen y dystysgrif at ddiben sy'n ymwneud â chofrestru o dan Ran 2 o'r Ddeddf neu os yw'r swydd yn dod o fewn rheoliad 5A o Reoliadau Ddeddf yr Heddlu 1997 (Cofnodion Troseddol) 2002, tystysgrif cofnod troseddol fanwl a ddyroddwyd o dan adran 113B o Ddeddf yr Heddlu 1997 ac sy'n cynnwys gwybodaeth am addasrwydd mewn perthynas ag oedolion hyglwyf (o fewn ystyr adran 113BB(2) o'r Ddeddf honno) ac, os yw'n gymwys, wybodaeth am addasrwydd mewn perthynas â phlant (o fewn ystyr adran 113BA(2) o'r Ddeddf honno); neu
(b)mewn unrhyw achos arall, tystysgrif cofnod troseddol a ddyroddwyd o dan adran 113A o Ddeddf yr Heddlu 1997.”;
(b)hepgorer paragraff 11.
(3) Yn Atodlen 3 (yr wybodaeth a'r ddogfennaeth sydd i fod ar gael mewn perthynas â gweithwyr gofal cartref) —
(a)yn lle paragraff 4 rhodder—
“4. Naill ai—
