Cofrestrau o hysbysiadau eithrio dros dro

12.  Mae'r manylion a ganlyn wedi eu rhagnodi ar gyfer pob cofnod mewn cofrestr a sefydlwyd ac sy'n cael ei gynnal o dan adran 232(1)(b) o'r Ddeddf o ran hysbysiad eithrio dros dro a gyflwynwyd o dan adran 62 neu 86 o'r Ddeddf—

(a)enw a chyfeiriad y person sy'n hysbysu'r awdurdod tai lleol o dan adran 62(1) neu 86(1) o'r Ddeddf;

(b)cyfeiriad yr HMO neu'r tŷ y mae'r awdurdod tai lleol wedi cyflwyno'r hysbysiad eithrio dros dro mewn perthynas ag ef ac unrhyw gyfeirnod a roddwyd iddo gan yr awdurdod tai lleol;

(c)crynodeb o effaith yr hysbysiad;

(ch)manylion unrhyw hysbysiadau eithrio dros dro blaenorol a gafodd eu cyflwyno mewn perthynas â'r un HMO neu dy am gyfnod sy'n union cyn yr hysbysiad eithrio dros dro cyfredol;

(d)datganiad o'r camau penodol y mae'r person y cyfeirir ato yn is-baragraff (a) yn bwriadu eu cymryd gyda golwg ar sicrhau nad yw'n ofynnol bellach i drwyddedu'r HMO neu'r tŷ;

(dd)y dyddiad pryd y cyflwynodd yr awdurdod tai lleol y rhybudd eithrio dros dro arno a'r dyddiad pan fydd y rhybudd yn peidio â bod mewn grym;

(e)gwybodaeth gryno ar unrhyw fater yn ymwneud â thrwyddedu'r HMO neu'r tŷ sydd wedi cael ei gyfeirio at dribiwnlys eiddo preswyl neu at y Tribiwnlys Tiroedd; ac

(f)gwybodaeth gryno ar unrhyw benderfyniad gan y tribiwnlysoedd y cyfeirir atynt yn is-baragraff (e) sy'n ymwneud â'r HMO neu'r tŷ trwyddedig, ynghyd â'r cyfeirnod a roddwyd i'r achos gan y tribiwnlys.
