Hysbysiadau i Fyrddau Iechyd Lleol
26.—(1) Rhaid i ymarferydd cymwysedig sydd wedi ei gynnwys yn rhestr gyfunol Bwrdd Iechyd Lleol hysbysu’r Bwrdd Iechyd Lleol hwnnw o fewn 14 o ddiwrnodau am unrhyw ddigwyddiad sy’n ei gwneud yn ofynnol newid yr wybodaeth y mae’n ofynnol i’r rhestr honno ei chynnwys mewn perthynas ag ef.
(2) Rhaid i ymarferydd cymwysedig sydd wedi ei gynnwys yn rhestr atodol Bwrdd Iechyd Lleol hysbysu’r Bwrdd Iechyd Lleol hwnnw o fewn 14 o ddiwrnodau am unrhyw newid i’w gyfeiriad preifat.
(3) Pan fo Bwrdd Iechyd Lleol yn cael hysbysiad gan ymarferydd cymwysedig yn unol â pharagraff (1) neu (2), rhaid iddo ddiwygio ei restr gyfunol cyn gynted ag y bo modd.
