Gofynion gweithdrefnol ar gyfer cyflwyno cynlluniau trwyddedau newydd

4.  Pan fo'n cyflwyno cynllun trwyddedau o dan adran 33(1) neu (2) o Ddeddf 2004, rhaid i'r Awdurdod Trwyddedau ddarparu i Weinidogion Cymru yr wybodaeth ganlynol —

(a)enw pob person sy'n awdurdod priffyrdd ar gyfer un neu fwy o'r strydoedd penodedig;

(b)beth yw amcanion yr Awdurdod Trwyddedau ar gyfer y cynllun trwyddedau hwnnw;

(c)sut mae'r Awdurdod Trwyddedau'n bwriadu sicrhau y bydd yn cydymffurfio â'r rhwymedigaeth a nodir yn rheoliad 40;

(ch)sut a phryd mae'r Awdurdod Trwyddedau'n bwriadu gwerthuso'r cynllun trwyddedau hwnnw er mwyn mesur a yw'r amcanion ar ei gyfer wedi'u bodloni;

(d)y costau a'r buddion (p'un a ydynt yn ariannol neu beidio) y mae'r Awdurdod Trwyddedau yn rhag-weld y byddant yn deillio o'r cynllun trwyddedau hwnnw;

(dd)y dystiolaeth a ystyriwyd gan yr Awdurdod Trwyddedau pan benderfynodd gynnwys unrhyw ddarpariaethau yn y cynllun trwyddedau o ran y ffioedd y caniateir eu codi, a'r rhesymau dros ei benderfyniad;

(e)ar neu ar ôl pa ddyddiad y mae'r Awdurdod Trwyddedau yn bwriadu bod y cynllun trwyddedau yn dod yn weithredol;

(f)manylion unrhyw drefniadau trosiannol y byddai'r Awdurdod Trwyddedau'n dymuno eu cymhwyso o ran pryd y byddai'r cynllun trwyddedau'n dod yn weithredol; ac

(ff)crynodeb o'r ymatebion a gafwyd i'r ymgynghoriad yr ymgymerwyd ag ef o dan reoliad 3 ac o'r newidiadau a wnaed i'r cynllun trwyddedau yn sgil yr ymgynghoriad hwnnw.
