Mae'r Rheoliadau hyn yn diwygio'r ffurflenni (dwyieithog) sydd i'w defnyddio i wneud cais am grant adleoli o dan adrannau 131 i 140 o Ddeddf Grantiau Tai, Adeiladu ac Adfywio 1996.
Mae'r ffurflen Gymraeg sy'n cael ei diwygio wedi'i nodi yn yr Atodlen i Reoliadau Grantiau Adleoli (Ffurflen Gais) (Ffurflen Gais Gymraeg) 1999 (O.S. 1999/2315 fel y'i diwygiwyd).
Mae'r ffurflen Saesneg sy'n cael ei diwygio wedi'i nodi yn yr Atodlen i Reoliadau Grantiau Adleoli (Ffurflen Gais) 1997 (O.S. 1997/2847 fel y'i diwygiwyd).
Mae Rheoliadau Grantiau Adleoli 1997 (O.S. 1997/2764) yn cymhwyso Rheoliadau Grantiau Adnewyddu Tai 1996 (O.S. 1996/2890) fel y maent yn effeithiol o dro i dro, gyda'r newidiadau sydd wedi'u rhagnodi. Gan hynny, mae diwygiadau i Reoliadau Grantiau Adnewyddu Tai 1996 gan Reoliadau Grantiau Adnewyddu Tai (Diwygio Rhif 2) (Cymru) 2001 (O.S. 2001/4007 (Cy.333)) wedi'i gwneud yn angenrheidiol diwygio'r ffurflenni, a'r Rheoliadau hyn sy'n gwneud y diwygiadau angenrheidiol.
Mae'r diwygiadau'n ymwneud â thaliadau arbennig a delir i ddioddefwyr amrywiolyn clefyd Creutzfeldt-Jakob a'u teuluoedd agos.
