18.—(1) Pan yw'r person wedi bwrw cyfnod ymsefydlu neu ran o gyfnod ymsefydlu, boed yng Nghymru neu Loegr, —
(a)enw'r corff priodol (o fewn ystyr “appropriate body” yn adran 19 o Ddeddf 1998);
(b)y dyddiad y dechreuodd y person y cyfnod ymsefydlu;
(c)os yw'n gymwys, nodyn i ddweud bod y person wedi cwblhau'r cyfnod ymsefydlu'n llwyddiannus, a dyddiad ei gwblhau;
(ch)os yw'n gymwys, nodyn i ddweud bod y person wedi methu â chwblhau'r cyfnod ymsefydlu'n llwyddiannus;
(d)os yw'n gymwys, nodyn i ddweud bod y cyfnod ymsefydlu wedi'i estyn ar gyfer y person, a chyfnod yr estyniad; a
(dd)os yw'n gymwys, nodyn i ddweud mai rhan yn unig o'r cyfnod ymsefydlu sydd wedi'i chwblhau gan y person, a'r cyfnod sydd wedi'i fwrw.
(2) Pan nad yw'r person wedi bwrw cyfnod ymsefydlu,
(a)a yw'r person yn esempt rhag y gofyniad i fwrw cyfnod ymsefydlu, a'r rheswm dros yr esemptiad; neu
(b)os nad oedd yn ofynnol bwrw cyfnod ymsefydlu ar yr adeg berthnasol, datganiad o'r ffaith honno(1).

Adeg gwneud y Gorchymyn hwn, nid oedd ymsefydlu'n ofynnol ar gyfer athrawon ac athrawesau yng Nghymru; nid oedd rheoliadau wedi'u gwneud o dan adran 19 o Ddeddf 1998 mewn perthynas â Chymru. Mae'r gofyniad ynghylch cyfnod ymsefydlu yn Lloegr yn gymwys ers 1 Medi 1999 (O.S. 1999/1065).
