8.  Rhaid i bob trwyddedai gofnodi, mewn perthynas â phob swp bwyd a arbelydrir ganddo, yr wybodaeth ganlynol—

(a)yn achos cyfleuster radioniwclid—

(i)o ran ffurfweddiad pob ffynhonnell o ymbelydredd ïoneiddio sydd ar gael i'w ddefnyddio yn y cyfleuster, unrhyw wybodaeth am ei sefyllfa sy'n dangos a oedd y swp bwyd yn agored iddo ac os felly pryd y digwyddodd hynny;

(ii)naill ai ar ba gyflymder y mae'r swp yn teithio drwy'r cyfleuster ac ar hyd pa lwybr y mae'r swp yn teithio tra bo'n mynd drwyddo neu'r amser y mae'r swp yn ei dreulio yn y parth arbelydru;

(b)yn achos ffynhonnell sy'n beiriant—

(i)lefel ei hynni;

(ii)ei cherrynt electron;

(iii)lled ei sganiwr;

(iv)nodweddion ei phelydr;

(v)onid oes ganddi ddyfais wasgaru, pa mor aml y mae ei phelydr yn sganio'r swp; a

(vi)ar ba gyflymder y mae'r swp yn teithio drwy'r cyfleuster.
