1. Bydd holiaduron ar-lein a gwefan yr Awdurdod sy’n lletya holiaduron ar-lein yn cynnwys gwybodaeth sy’n ymwneud â’r cyfrifiad ac o ran llenwi a chyflwyno holiaduron ar-lein, gan gynnwys gwybodaeth am y canlynol—
(a)dechrau holiadur ar-lein,
(b)y personau y mae rhaid iddynt lenwi ffurflen y cyfrifiad,
(c)aelodau o’r aelwyd yn ateb y rhannau o holiadur sy’n berthnasol iddynt hwy,
(d)pryd y dylid llenwi ffurflen y cyfrifiad,
(e)sut y bydd yr Awdurdod yn defnyddio ac yn diogelu gwybodaeth sy’n ymwneud â pherson rhagnodedig,
(f)ble a sut y gellir dod o hyd i gymorth er mwyn llenwi holiadur ar-lein, ac
(g)natur fandadol y cyfrifiad yn ei gyfanrwydd, ond natur wirfoddol cwestiynau penodol o fewn y cyfrifiad.
