Part 2 of the Act provides for the registration and inspection of establishments and agencies by the registration authority (the Welsh Ministers). Part 2 of the Act also provides powers for the Welsh Ministers to make regulations governing the conduct of establishments and agencies. PART 39A certificate of registration must contain the following particulars—athe name, address and telephone number of the appropriate office of the registration authority;bthe name and address of the registered provider;cwhere the person is an organisation, the name of the responsible individual;dthe name of the registered manager;ethe description of the private dental practice;fwhere the registration is subject to conditions, details of the condition;gthe date of registration;ha statement that if a private dental practice is not carried on in accordance with the relevant requirements and any conditions, the registration is liable to be cancelled by the registration authority;ia statement that the certificate relates only to the person to whom it is issued by the registration authority and is not capable of being transferred to another person.10If the registration of a person in respect of a private dental practice is cancelled, that person must, not later than the day on which the decision or order cancelling the registration takes effect, return the certificate of registration to the registration authority by—adelivering it to the appropriate office of the registration authority; orbsending it to the appropriate office of registration authority by registered post or recorded delivery.11A failure to comply with regulation 10 is an offence.Reg. 9 in force at 1.4.2017, see reg. 1Reg. 10 in force at 1.4.2017, see reg. 1Reg. 11 in force at 1.4.2017, see reg. 1
