Where an application for planning permission is made, the applicant must certify, in a form published by the Welsh Ministers or in a form substantially to the like effect, that the requirements of article 10 have been satisfied.

If an applicant has cause to rely on paragraph (5) of article 10, the certificate must state the relevant circumstances.
