Parhad trwydded, gwneud cais am drwydded a thelerau trwydded
5.—(1) Rhaid i berson sy'n dymuno gwneud cais am drwydded gwblhau a chyflwyno cais i Weinidogion Cymru ym mha bynnag ffurf sy'n ofynnol gan Weinidogion Cymru.
(2) Rhaid i gais yn unol â pharagraff (1) gael ei gyflwyno ynghyd â pha bynnag brawf adnabod a phrawf cyfeiriad sy'n ofynnol gan Weinidogion Cymru.
(3) Ceir gwneud cais yn unol â pharagraff (1) ar, neu ar ôl, 1 Gorffennaf yn yr un flwyddyn ag y bwriedir i'r drwydded gychwyn.
