ATODLEN 3MATERION ADDYSGOL Y MAE AWDURDODAU CYFRIFOL I'W HYSTYRIED.
1. Hanes addysgol y plentyn.
2. Yr angen i sicrhau dilyniant yn addysg y plentyn a hybu cyrhaeddiad addysgol y plentyn.
3. Yr angen i ddynodi unrhyw angen addysgol a eill fod gan y plentyn ac i weithredu i ddiwallu'r angen hwnnw.
4. Yr angen i wneud unrhyw asesiad mewn perthynas ag unrhyw angen addysgol arbennig o dan Ddeddf Addysg 1996(1) ac i ddiwallu unrhyw anghenion o'r fath a ddynodir mewn datganiad anghenion addysgol arbennig a wneir o dan adran 324 o'r Ddeddf honno.
5. O ystyried yr wybodaeth sydd ar gael ym mharagraffau 1 i 4 o'r Atodlen hon, a fydd anghenion addysgol y plentyn yn cael eu diwallu yn y lleoliad arfaethedig.
