Digwyddiadau hysbysadwy
27.—(1) Os bydd unrhyw un o'r digwyddiadau a restrir yng ngholofn 1 o'r tabl yn Atodlen 4 (“y tabl”) yn digwydd mewn cysylltiad ag asiantaeth, rhaid i'r person cofrestredig hysbysu'r person a nodir yn ddi-oed ynglyn â'r digwyddiad yng ngholofn 2 o'r tabl.
(2) Rhaid cadarnhau yn ysgrifenedig unrhyw hysbysiad sy'n cael ei roi ar lafar yn unol â'r rheoliad hwn o fewn 10 o ddiwrnodau gwaith.
(3) Yn y tabl —
ystyr “awdurdod perthnasol” yw —
(a)yr awdurdod lleol y mae'r asiantaeth wedi'i leoli yn ei ardal, a
(b)o ran plentyn sydd wedi marw neu sydd wedi'i anafu'n ddifrifol wrth gael gwasanaethau cymorth mabwysiadu, unrhyw awdurdod lleol arall y mae'r asiantaeth yn darparu gwasnaethau cymorth mabwysiadu ar ei ran i'r plentyn hwnnw o dan reoliadau a wnaed o dan adran 3(4)(b) o Ddeddf 2002,
(c)ystyr “Bwrdd Iechyd Lleol” ac “Ymddiriedolaeth Gofal Sylfaenol” yw'r Bwrdd Iechyd Lleol neu'r Ymddiriedolaeth Gofal Sylfaenol yr oedd y plentyn sydd wedi marw neu sydd wedi'i anafu'n ddifrifol wrth gael gwasanaethau cymorth mabwysiadu yn byw yn ei ardal adeg y digwyddiad.
