Rheoliad 36(2)
ATODLEN 6CYNLLUN LLEOLIAD
1. Statws y plentyn a ph'un a leolwyd ef o dan orchymyn lleoliad neu gyda chydsyniad ei rieni gwaed.
2. Y trefniadau ar gyfer paratoi'r plentyn a'r darpar fabwysiadydd ar gyfer y lleoliad.
3. Y dyddiad y bwriedir lleoli'r plentyn i'w fabwysiadu gyda'r darpar fabwysiadydd.
4. Y trefniadau ar gyfer adolygu'r lleoliad.
5. A fwriedir cyfyngu ar gyfrifoldeb rhiant y darpar fabwysiadydd ac os felly i ba raddau y bydd y cyfyngu.
6. Y gwasanaethau cymorth mabwysiadu y mae'r awdurdod lleol wedi penderfynu eu darparu ar gyfer y plentyn a'r teulu mabwysiadol, sut y darperir hwy a chan bwy (os yw'n gymwys).
7. Y trefniadau a wnaeth yr asiantaeth fabwysiadu er mwyn caniatáu cyswllt â'r plentyn i unrhyw berson, ffurf y cyswllt a'r trefniadau ar gyfer cefnogi cyswllt ac enw a manylion cyswllt y person sy'n gyfrifol am hwyluso'r trefniadau cyswllt (os yw'n gymwys).
8. Y dyddiad pan roddir llyfr stori bywyd a llythyr am fywyd yn nes ymlaen i'r darpar fabwysiadydd neu i'r plentyn.
9. Manylion unrhyw drefniadau y mae angen eu gwneud.
10. Manylion cyswllt gweithiwr cymdeithasol y plentyn, gweithiwr cymdeithasol y darpar fabwysiadydd a chysylltiadau y tu allan i oriau.
