Gofyniad i ddarparu paratoad ar gyfer mabwysiadu

25.—(1) Pan fydd yr asiantaeth fabwysiadu yn ystyried a yw person yn addas i fabwysiadu plentyn, rhaid i'r asiantaeth wneud trefniadau i'r darpar fabwysiadydd gael y paratoad hwnnw ar gyfer mabwysiadu y mae'r asiantaeth yn ystyried ei bod yn briodol.

(2) Nid yw paragraff (1) yn gymwys os yw'r asiantaeth fabwysiadu wedi'i bodloni bod y gofynion a nodir yn y paragraff hwnnw wedi cael eu cyflawni o ran y darpar fabwysiadydd gan asiantaeth fabwysiadu arall.

(3) Pan na fydd y darpar fabwysiadydd yn barod i ymgymryd â'r paratoad ar gyfer mabwysiadu y mae'r asiantaeth fabwysiadu yn ystyried ei fod yn briodol yn ei achos ef, caniateir i'r asiantaeth wrthod mynd ymlaen â chais y darpar fabwysiadydd am asesiad o'i addasrwydd.

(4) Ym mharagraff (1) mae “paratoad ar gyfer mabwysiadu” yn cynnwys darparu gwybodaeth i ddarpar fabwysiadydd ynghylch —

(a)ystod oedran, rhyw ac anghenion tebygol a chefndir plant y gellid eu lleoli ar gyfer mabwysiadu gan yr asiantaeth fabwysiadu;

(b)arwyddocâd a goblygiadau cyfreithiol mabwysiadu i blentyn a'i deulu;

(c)cyswllt rhwng plentyn a'i rieni naturiol a pherthnasau eraill pan awdurdodir lleoli plentyn ar gyfer mabwysiadu neu pan gaiff y plentyn ei fabwysiadu;

(ch)y sgiliau sydd eu hangen i fod yn rhiant mabwysiadol;

(d)safbwynt y plentyn a theulu'r plentyn;

(dd)gweithdrefnau'r asiantaeth fabwysiadu o ran asesu darpar fabwysiadwyr a lleoli plentyn ar gyfer mabwysiadu; a

(e)y weithdrefn o ran mabwysiadu a lleoli ar gyfer mabwysiadu.
