1.  Yr wybodaeth ganlynol am y disgybl —

(a)Rhif unigryw cyfredol y disgybl, ac os bu gan yr ysgol rif unigryw disgybl blaenorol ar gyfer y disgybl hwnnw, y Rhif blaenorol;

(b)cyfenw;

(c)enw cyntaf y disgybl, neu bob enw cyntaf os oes mwy nag un;

(ch)enw canol y disgybl, neu bob enw canol os oes mwy nag un;

(d)rhyw;

(dd)dyddiad geni;

(e)grŵp ethnig;

(f)hunaniaeth genedlaethol;

(ff)dyddiad derbyn y disgybl i'r ysgol; a

(g)grŵp blwyddyn y Cwricwlwm Cenedlaethol yr addysgir y disgybl ynddo.
