7. Yr wybodaeth ganlynol ynghylch pob person a gyflogir yn yr ysgol —
(a)enw'r person ac unrhyw enw blaenorol a fu ganddo;
(b)rhyw'r person, ei ddyddiad geni, ei rif Yswiriant Cenedlaethol ac ym mha swyddogaeth y caiff ei gyflogi;
(c)yn achos pob athro, ei gymwysterau a datganiad yn dweud ai ef yw'r pennaeth neu a yw'n athro llawnamser neu ran-amser; ac
(ch)cadarnhad ei fod ef wedi derbyn Tystysgrif Ddatgelu ar y lefel briodol gan y Swyddfa Cofnodion Troseddol yn cadarnhau ei fod yn addas i weithio gyda phlant.
