4. Rhaid i berchennog ysgol gofrestredig gyflwyno i'r awdurdod cofrestru cyn pen tri mis ar ôl derbyn disgyblion, neu un disgybl os yw'r disgybl hwnnw o fewn adran 463(1)(b) o Ddeddf 1996, ffurflen ysgrifenedig rhaid ei bod yn cynnwys —
(a)yr wybodaeth a bennir yn Rhan 3 o'r Atodlen; a
(b)tystysgrif wedi'i llofnodi gan y perchennog, neu gan berson a awdurdodwyd ganddo i roi'r dystysgrif ar ei ran, bod y datganiadau a wnaed yn y ffurflen yn gywir hyd eithaf ei wybodaeth a'i gred.
