Mae Cynulliad Cenedlaethol Cymru yn gwneud y Gorchymyn canlynol drwy arfer y pwerau a roddwyd i Ysgrifennydd Gwladol Cymru gan adran 126(3) o Ddeddf y Gwasanaeth Iechyd Gwladol 1977(1) ac adran 5(1), a pharagraff 29(1) o Atodlen 2 i Ddeddf y Gwasanaeth Iechyd Gwladol a Gofal Cymunedol 1990(2) a phob pŵer arall sy'n galluogi'r Ysgrifennydd Gwladol yn y cyswllt hwnnw ac a freiniwyd bellach yng Nghynulliad Cenedlaethol Cymru(3), gan ei fod yn barnu ei bod yn briodol gwneud y Gorchymyn hwn(4) er lles y gwasanaeth iechyd, ac ar ôl cwblhau'r ymgynghoriad a ragnodwyd o dan baragraff 29(3) o Atodlen 2 i'r Ddeddf olaf(5):
