in paragraph (a), for “3, 15 or 15A” substitute “
3, 14A, 15, 15A or 19A
”
; and

for paragraph (d) substitute—

in the list of visiting medical practitioners from relevant European States, the names of persons entitled to be registered under Schedule 2A.
