Schedule 1: Information to be included with notification of conviction

  1. Paragraph (1) sets out that the information, in accordance with the TCA, set out in the subsequent paragraphs of this schedule should be included when notifying EU Member States of an individual’s convictions. The information mentioned in paragraphs (4), (8) to (12), (15) and (18) is only required when recorded on the criminal records database.
  2. The information required is as follows:
    1. Paragraph (2): The individual’s name
    2. Paragraph (3): Any previous name of the individual
    3. Paragraph (4): Any other name used by the individual (when available on the criminal records database)
    4. Paragraph (5): The individual’s gender
    5. Paragraph (6): The individual’s date and place of birth
    6. Paragraph (7): The individual’s nationality or nationalities
    7. Paragraph (8): The names of the individual’s parents (when available on the criminal records database)
    8. Paragraph (9): The number of any passport held by the individual (when available on the criminal records database)
    9. Paragraph (10): The issue number and description of any identity document (when available on the criminal records database)
    10. Paragraph (11): The individual’s fingerprints (when available on the criminal records database)
    11. Paragraph (12): A photograph or other image of the individual’s face (when available on the criminal records database)
    12. Paragraph (13): The date of the conviction
    13. Paragraph (14): In the case of a conviction by or before a court, the court by or before which the individual was convicted. In any other case, the person or description of person by or before which the individual was convicted
    14. Paragraph (15): The reference number of the conviction (when available on the criminal records database)
    15. Paragraph (16): The offence of which the individual was convicted.
    16. Paragraph (17): The date on which the offence was committed.
    17. Paragraph (18): The place where the offence was committed (when available on the criminal records database).
    18. Paragraph (19): Any sentence in respect of the offence.
    19. Paragraph (20): Any other order made in respect of the offence.
