“15. Officer of a Health Authority, a Special Health Authority or any other body constituted under the M2National Health Service Act 1977 or the M3National Health Service and Community Care Act 1990The Health Authority, Special Health Authority or other body.
15A. Officer of a Health Board, the Common Services Agency for the Scottish Health Service or any other body constituted under the M4National Health Service (Scotland) Act 1978The Health Board, Agency or other body.”,

and, in paragraph 16, for the entry in the second column substitute “ The Health Authority or Health Board for the area for which the services are provided. ”
