(a) on the first page— (i) for “European Union (1)” substitute “United Kingdom”; (ii) in each place the words occur, for “competent authority” substitute “CAA”; (iii) for “EASA Form 153” substitute “CAA Form 153”; (iv) omit footnote (1);

(b) on the second page— (i) next to “Signed”, for “competent authority” substitute “CAA”; (ii) for “Member State/EASA” substitute “CAA”; (iii) for “EASA Form 153” substitute “CAA Form 153”; (iv) in footnotes (2), (4), (5) and (6), for “competent authority” substitute “CAA”.
