10. Rhaid i gais i gofrestru neu gais am drwydded gynnwys datganiad wedi ei gwblhau ar y ffurf ganlynol—
“Rwyf i/rydym ni yn datgan bod yr wybodaeth a gynhwysir yn y cais hwn yn gywir hyd eithaf fy ngwybodaeth/ein gwybodaeth. Rwyf i/rydym ni yn deall fy mod/ein bod yn cyflawni trosedd os byddaf/byddwn yn darparu unrhyw wybodaeth i’r awdurdod trwyddedu mewn cysylltiad ag unrhyw rai o’i swyddogaethau o dan Ran 1 o Ddeddf Tai (Cymru) 2014 sy’n anwir neu’n gamarweiniol ac y gwn/gwyddom ei bod yn anwir neu’n gamarweiniol neu os byddaf/byddwn yn ddi-hid ynghylch pa un a yw’n anwir neu’n gamarweiniol.”
