RHAN 2Gwybodaeth i’w chynnwys mewn asesiad addysg cyfrwng Cymraeg
Rhaid cynnwys yr wybodaeth ganlynol mewn unrhyw asesiad addysg cyfrwng Cymraeg—
(a)datganiad cryno o ddiben yr asesiad addysg cyfrwng Cymraeg;
(b)datganiad bod addysg cyfrwng Cymraeg yn opsiwn i blant Cymraeg a di-Gymraeg;
(c)enwau a chyfeiriadau ysgolion a gynhelir yn ardal yr awdurdod lleol sy’n darparu addysg cyfrwng Cymraeg;
(d)disgrifiad o faint o addysgu sy’n digwydd drwy gyfrwng y Gymraeg ym mhob un o’r ysgolion a gynhelir sy’n darparu addysg cyfrwng Cymraeg yn ardal yr awdurdod lleol;
(e)esboniad cryno o’r cyfleoedd i ddisgyblion drosglwyddo o ysgolion cynradd a gynhelir sy’n darparu addysg cyfrwng Cymraeg i ysgolion uwchradd a gynhelir sy’n darparu addysg cyfrwng Cymraeg; ac
(f)dyddiad cau’r asesiad addysg cyfrwng Cymraeg.
