Cofnodion ychwanegol

2.—(1) Ar gyfer pob cyflenwad unigol, rhaid iddo gofnodi pob un o'r canlynol o fewn 28 diwrnod ar ôl iddynt ddigwydd neu gael eu cwblhau—

(a)cynllun a disgrifiad o'r cyflenwad;

(b)y rhaglen ar gyfer monitro'r cyflenwad;

(c)yr asesiad risg;

(ch)dyddiad, canlyniadau a lleoliad unrhyw samplu a dadansoddi mewn perthynas â'r cyflenwad dan sylw, a'r rheswm dros gymryd y sampl;

(d)canlyniadau unrhyw ymchwiliad a gynhelir yn unol â'r Rheoliadau hyn;

(dd)unrhyw awdurdodiad;

(e)unrhyw hysbysiadau a gyflwynir o dan adran 80 o Ddeddf y diwydiant Dŵr 1991, neu reoliad 18;

(f)unrhyw weithred y cytunir sydd i'w chyflawni gan unrhyw berson o dan y Rheoliadau hyn;

(ff)unrhyw gais a wneir i'r awdurdod lleol am samplu a dadansoddi, cynnal asesiad risg neu roi cyngor;

(g)crynodeb o unrhyw gyngor a roddir mewn perthynas â'r cyflenwad.

(2) Rhaid iddo ddal ei afael yn yr asesiad risg a'r cofnodion samplu a dadansoddi am o leiaf ddeng mlynedd ar hugain, a phob cofnod arall o dan y paragraff hwn am o leiaf bum mlynedd.
