ATODLEN 3Y COFNODION SYDD I'W CYNNAL
1. Enw, cyfeiriad a rhif teleffon y person cofrestredig, yr unigolyn cyfrifol a phob person arall sy'n byw, yn gweithio neu a gyflogir yn y fangre berthnasol .
2. Enw, cyfeiriad cartref a rhif teleffon unrhyw berson arall a ddaw i gysylltiad â'r plant perthnasol yn rheolaidd heb oruchwyliaeth.
3. Enw, cyfeiriad cartref, dyddiad geni a rhyw pob plentyn perthnasol.
4. Mewn perthynas â phob plentyn perthnasol, enw, cyfeiriad a rhif teleffon rhiant.
5. Mewn perthynas â phob plentyn perthnasol, enw a chyfeiriad yr ymarferydd meddygol cofrestredig y cofrestrwyd y plentyn gydag ef.
6. Cofnod dyddiol o enwau'r plant perthnasol, oriau eu presenoldeb ac enwau'r personau a fu'n gofalu amdanynt.
7. Cofnod o ddamweiniau, salwch difrifol a digwyddiadau arwyddocaol eraill a ddigwyddodd yn fangre berthnasol ac a effeithiodd ar les plant perthnasol.
8. Cofnod o unrhyw gynnyrch meddyginiaethol a gafodd ei weini i blentyn perthnasol ar y fangre berthnasol, gan gynnwys y dyddiad a'r amgylchiadau a chan bwy y cafodd y feddyginiaeth ei gweini, a chan gynnwys unrhyw gynhyrchion meddyginiaethol y caniateir i'r plentyn eu gweini ei hunan, ynghyd â chofnod o ganiatâd y rhiant.
9. Unrhyw anghenion dietegol arbennig, neu anghenion iechyd arbennig neu alergedd unrhyw blentyn perthnasol.
10. Datganiad o'r weithdrefn sydd i'w dilyn os digwydd tân neu ddamwain.
11. Datganiad o'r weithdrefn sydd i'w dilyn os oes gan riant gwyn ynglŷn â'r gwasanaeth a ddarperir gan y person cofrestredig.
