Rheoliad 3

ATODLEN 1CYMHWYSO RHAN II O DDEDDF SAFONAU GOFAL 2000 I BERSONAU SY'N DARPARU GWASANAETHAU DEINTYDDOL PREIFAT

At ddibenion yr Atodlen hon rhaid deall cyfeiriadau yn Rhan II o'r Ddeddf —

(a)at sefydliad neu asiantaeth fel cyfeiriadau at berson sy'n darparu gwasanaethau deintyddol preifat;

(b)at gyfleusterau neu wasanaethau a ddarperir mewn sefydliad fel cyfeiriadau at gyfleusterau neu wasanaethau a ddarperir gan berson sy'n darparu gwasanaethau deintyddol;

(c)at fangre a ddefnyddir yn sefydliad neu at ddibenion asiantaeth fel cyfeiriadau at fangre a ddefnyddir at ddibenion darparu gwasanaethau deintyddol.

(ch)at gynnal neu reoli sefydliad neu asiantaeth fel cyfeiriadau at ddarparu gwasanaethau deintyddol preifat.

Rheoliad 13

ATODLEN 2GWYBODAETH A DOGFENNAU A FYDD AR GAEL YNGHYLCH PERSONAU COFRESTREDIG

1.  Prawf o bwy yw'r person gan gynnwys ffotograff diweddar.

2.  Tystysgrif record droseddol fanwl y mae llai na thair blynedd wedi mynd heibio ers pan ddyroddwyd hi ddiwethaf.

3.  Datganiad gan y person cofrestredig ei fod wedi cael ei gynnwys ar y gofrestr deintyddion.

4.  Rhif cofrestriad proffesiynol y person cofrestredig neu Dystysgrif Deintydd AEE sy'n ymweld.

5.  Tystysgrif yswiriant ar gyfer y person cofrestredig mewn perthynas ag unrhyw rwymedigaeth y mae'n bosibl i'r person fynd iddi mewn perthynas â darparu gwasanaethau deintyddol o ran marwolaeth, anaf, atebolrwydd i'r cyhoedd, difrod neu unrhyw golled arall.
