Rheoliad 14

ATODLEN 4Gwybodaeth i'w chynnwys mewn datganiad o gydymffurfedd

1.  Enw a chyfeiriad y gweithredwr busnes sy'n gweithgynhyrchu neu'n mewnforio'r deunyddiau neu'r eitemau plastig neu'r sylweddau a fwriedir ar gyfer gweithgynhyrchu'r deunyddiau neu'r eitemau hynny.

2.  Enw'r deunyddiau, yr eitemau neu'r sylweddau a fwriedir ar gyfer eu gweithgynhyrchu.

3.  Dyddiad y datganiad.

4.  Cadarnhad bod y deunyddiau neu'r eitemau plastig yn bodloni'r gofynion perthnasol a osodir yn y Gyfarwyddeb ac yn Rheoliad 1935/2004.

5.  Gwybodaeth ddigonol mewn perthynas â'r sylweddau a ddefnyddir y mae cyfyngiadau neu fanylebau yn bodoli ar eu cyfer o dan y Gyfarwyddeb er mwyn caniatáu gweithredwyr busnes derbyn i sicrhau cydymffurfedd â hwy.

6.  Gwybodaeth ddigonol mewn perthynas â'r sylweddau sy'n ddarostyngedig i gyfyngiad ar fwyd, a geir drwy gyfrwng data arbrofol neu gyfrifo theoretig ynghylch lefel eu hymfudiad penodol a, phan fo hynny'n briodol, feini prawf purdeb yn unol â'r Cyfarwyddebau purdeb i alluogi defnyddiwr y deunyddiau neu'r eitemau plastig i gydymffurfio â'r darpariaethau Cymunedol perthnasol neu, yn absenoldeb y cyfryw ddarpariaethau, â darpariaethau cenedlaethol sy'n gymwys i fwyd.

7.  Manylebau ynghylch defnyddio'r deunydd neu'r eitem plastig, megis —

(a)y mathau o fwyd y bwriedir iddo fod mewn cyffyrddiad â hwy;

(b)amser a thymheredd ei drin a'i storio mewn cyffyrddiad â'r bwyd;

(c)cymhareb arwynebedd cyffyrddiad â bwyd i gyfaint a ddefnyddir i gadarnhau cydymffurfedd y deunydd neu'r eitem plastig.

8.  Cadarnhad, pan ddefnyddir gwahanfur swyddogaethol plastig mewn plastig amlhaenog, bod y deunydd neu'r eitem plastig yn cydymffurfio â gofynion paragraffau 2 i 4 o Erthygl 7a o'r Gyfarwyddeb.
