3.—(1) Sefydlir yr ymddiriedolaeth at y dibenion a bennir yn adran 18(1) o'r Ddeddf.

(2) Swyddogaethau'r ymddiriedolaeth fydd darparu nwyddau a gwasanaethau (gan gynnwys lle mewn ysbyty a gwasanaethau iechyd cymunedol) o

(a)Ysbyty Glan Clwyd, Y Rhyl, Sir Ddinbych, LL18 5UJ;

(b)Ysbyty Wrecsam Maelor, Ffordd Croesnewydd, Wrecsam, LL13 7TD;

ac ysbytai a mangreoedd cysylltiedig.
