Disgyblion sydd wedi eu Gwahardd

1.  Yr wybodaeth ganlynol am bob disgybl sydd wedi ei wahardd yn barhaol o'r ysgol ac yr oedd ei ddyddiad gwahardd parhaol yn y flwyddyn a ddaeth i ben ar 31 Awst cyn y dyddiad y gwneir y cais am wybodaeth—

(a)Rhif unigryw cyfredol disgybl;

(b)cyfenw;

(c)enw cyntaf y disgybl, neu bob enw cyntaf os oes mwy nag un;

(ch)enw canol y disgybl, neu bob enw canol os oes mwy nag un;

(d)rhyw;

(dd)dyddiad geni; ac

(e)y dyddiad y dechreuodd y gwaharddiad parhaol.
