Cardiau hawl

11.—(1) Pan gaiff Bwrdd Iechyd Lleol ei fodloni bod person yn glaf cymwys, bydd yn dyroddi cerdyn i'r person hwnnw (y cyfeirir ato yn y Rheoliadau hyn fel “cerdyn hawl”) a fydd yn ddilys am unrhyw gyfnod y bydd y Bwrdd Iechyd Lleol yn penderfynu arno.

(2) Mae cerdyn hawl a ddyroddir yn rhoi i'r claf cymwys yr hawl i gael cyffuriau a chyfarpar yn ddi-dâl o dan baragraffau (1)(b) a (4)(b)(i) o reoliad 3 a pharagraff (1) o reoliad 4.

(3) At ddibenion y Rheoliadau hyn ystyr “claf cymwys” yw person —

(a)sy'n preswylio yng Nghymru; a

(b)sy'n derbyn gwasanaethau meddygol sylfaenol o dan Ran 1 o'r Ddeddf gan ddarparydd a gontractiwyd i ddarparu gwasanaethau o'r fath gydag Ymddiriedolaeth Gofal Sylfaenol; ac

mae'n cynnwys person sy'n gweithredu ar ran person o'r fath.
