1.  Rwyf wedi'm bodloni bod math y ci a math ei fam wedi'u nodi ym mharagraff 4 a 5 yn eu trefn o Ran 1.

2.  Rwyf wedi gweld y dystiolaeth a gyflwynwyd yn unol â paragraffau 7-9 o Ran 1 uchod, fod y ci yn debygol o gael ei ddefnyddio at waith mewn cysylltiad ag un o'r dibenion a bennir ym mharagraff 6 o Ran 1 uchod.

3.  Yn unol ag adran 6(4) o Ddeddf Lles Anifeiliaid 2006 rwyf yn ardystio fod yr amodau yn adran 6(5) a (6) o'r Ddeddf wedi'u cyfarfod o ran y ci a gyflwynwyd i mi a pharthed yr hwn y cafodd rhan 1 uchod ei gwblhau.
