Y gofrestr adran 121B

5.—(1) Yn ychwanegol at yr wybodaeth a ragnodir yn rheoliad 3, rhaid i'r awdurdod gynnwys yn ei gofrestr adran 121B—

(a)y dyddiad erbyn pryd y mae'r awdurdod yn bwriadu penderfynu'r cais (os yw'n gymwys);

(b)y dyddiad y mae'r awdurdod yn penderfynu'r cais;

(c)dyfarniad yr awdurdod;

(ch)y dyddiad y mae gorchymyn yn cael ei wneud, a phan fo'n gymwys, yn cael ei gadarnhau;

(d)pan fo'r awdurdod yn cael ei hysbysu bod y ceisydd wedi apelio i'r Cynulliad Cenedlaethol yn unol ag adran 121D o Ddeddf 1980, datganiad i'r perwyl hwnnw; ac, ar ôl cael ei hysbysu o ddyfarniad y Cynulliad Cenedlaethol, datganiad o ganlyniad yr apêl; ac

(dd)pan fo'n ymarferol, dyddiad, amser a lleoliad unrhyw wrandawiad neu ymchwiliad arfaethedig.
