Datganiad o ddiben

4.—(1) Rhaid i'r person cofrestredig lunio datganiad ysgrifenedig (y cyfeirir ato yn y Rheoliadau hyn fel “y datganiad o ddiben” (“the statement of purpose”)) mewn perthynas â'r cartref gofal a fydd yn cynnwys—

(a)datganiad o nodau ac amcanion y cartref gofal;

(b)datganiad ynghylch y cyfleusterau a'r gwasanaethau sydd i'w darparu gan y person cofrestredig ar gyfer y defnyddwyr gwasanaeth; ac

(c)datganiad ynghylch y materion a restrir yn Atodlen 1.

(2) Rhaid i'r person cofrestredig ddarparu copi o'r datganiad o ddiben i swyddfa briodol y Cynulliad Cenedlaethol a threfnu ei fod ar gael i'w archwilio pan wneir cais amdano ar unrhyw adeg resymol gan unrhyw ddefnyddiwr gwasanaeth ac unrhyw gynrychiolydd i ddefnyddiwr gwasanaeth.

(3) Ni fydd dim yn rheoliad 16(1) neu 24(1) yn ei gwneud yn ofynnol nac yn awdurdodi'r person cofrestredig i dorri'r canlynol, neu i beidio â chydymffurfio â hwy—

(a)unrhyw ddarpariaeth arall yn y Rheoliadau hyn; neu

(b)yr amodau sydd mewn grym am y tro mewn perthynas â chofrestru'r person cofrestredig o dan Ran II o'r Ddeddf.
