ATODLEN 4YR WYBODAETH SYDD I'W CHYNNWYS MEWN ADRODDIAD ASESU
1. Nodi nodweddion yr organedd derbyn sy'n berthnasol i'r asesiad o'r organeddau a addaswyd yn enetig perthnasol.
2. Disgrifiad o'r ffordd y mae'r addasiad genetig yn effeithio ar nodweddion yr organeddau.
3. Nodi unrhyw risgiau hysbys o newid i'r amgylchedd a allai ddeillio o ollwng i'r amgylchedd yr organedd derbyn nas addaswyd.
4. Asesiad ynghylch a yw'r addasiad genetig wedi'i nodweddu'n ddigonol at ddibenion gwerthuso unrhyw risgiau i iechyd dynol a'r amgylchedd.
5. Dynodi unrhyw risgiau newydd i iechyd dynol a'r amgylchedd allai ddeillio o ollwng yr organeddau perthnasol a addaswyd yn enetig o'i gymharu â gollwng yr organedd cyfatebol nas addaswyd yn enetig, yn seiliedig ar yr asesiad risg amgylcheddol.
6. Diweddglo sy'n mynd i'r afael â defnydd arfaethedig o'r cynnyrch, rheoli risg a'r cynllun monitro arfaethedig, ac sy'n datgan a ddylid marchnata'r organeddau perthnasol a addaswyd yn enetig ac o dan ba amodau, neu na ddylid eu marchnata, gan gynnwys rhesymau pam y daethpwyd i'r casgliad hwnnw, ac a fwriedir gofyn am farn awdurdodau cymwys yr Aelod-wladwriaethau eraill a'r Comisiwn ar agweddau penodedig yr asesiad risg amgylcheddol a beth yw'r agweddau hynny.
