Swyddogaethau cynghorwyr personol

12.—(1) Bydd gan gynghorydd personol y swyddogaethau canlynol(1)—

(a)mewn perthynas â phlant cymwys a pherthnasol, y swyddogaethau a restrir ym mharagraff (2); a

(b)mewn perthynas â chyn blant perthnasol, y swyddogaethau a restrir ym mharagraff (2) heblaw is-baragraff (b).

(2) Dyma'r swyddogaethau —

(a)rhoi cyngor (gan gynnwys cyngor ymarferol) a chymorth iddynt;

(b)cymryd rhan yn eu hasesiadau ac yng ngwaith paratoi eu cynlluniau cam nesaf;

(c)cymryd rhan mewn adolygiadau o'u cynlluniau cam nesaf;

(ch)cysylltu â'r awdurdod lleol cyfrifol wrth i'r cynllun cam nesaf gael ei weithredu;

(d)cyd-drefnu gwaith darparu gwasanaethau iddynt, a chymryd camau rhesymol i sicrhau eu bod yn defnyddio'r gwasanaethau hynny;

(dd)parhau'n gyfarwydd â'u cynnydd a'u llesiant; ac

(e)cadw cofnod ysgrifenedig o unrhyw un o'i gysylltiadau â hwy.

Yn ychwanegol gall yr awdurdod lleol cyfrifol ei gweld yn dda trefnu i'r cynghorydd personol weithredu ar ei ran wrth ddarparu gwasanaethau eraill yn unol ag adran 17(5)(b) o'r Ddeddf.
