3.—(1) Y cyrff yn y Gwasanaeth Iechyd Gwladol a ragnodir at ddibenion adran 31 o'r Ddeddf yw —
(a)Awdurdod Iechyd(1); a
(b)Ymddiriedolaeth Gwasanaeth Iechyd Gwladol(2);
(2) Yr awdurdodau lleol a ragnodir at ddibenion adran 31 o'r Ddeddf yw —
(a)cyngor sir; a
(b)cyngor bwrdeistref sirol.
