11.  Ar ôl nodyn 2, mewnosodwch –

“2A.  Os oes gennych rif yswiriant gwladol, byddwch cystal â darparu tystiolaeth ei fod yn perthyn i chi, megis cerdyn rhif, datganiadau cyflog, ffurflenni budd-dâl neu ddogfen swyddogol arall sy'n dwyn eich enw a'ch rhif yswiriant gwladol gyda'i gilydd. Os ydych wedi colli'ch rhif yswiriant gwladol neu os na ddyrannwyd un i chi, byddwch cystal â darparu copi o'ch datganiad cyflog neu'ch ffurflen fudd-dâl fel y gellir olrhain eich rhif yswiriant gwladol neu fel y gellir gwneud cais am rif yswiriant gwladol.

Mae Adran Dai y Cyngor yn gallu rhoi cyngor i chi am ba ddogfennau y dylech eu darparu a hefyd sut i wneud cais am rif yswiriant gwladol newydd. Llenwch weddill y ffurflen a gofyn am eu cyngor pan fyddwch yn anfon y cais atynt”.
