9.—(1) Each of the forms listed in subparagraphs (a) to (i) and set out in Schedule 5 to this Order is prescribed as the version partly in Welsh and partly in English, of the forms in the Appendix to Schedule 3 to the 2006 Rules, which must be used at a principal area election taken with a relevant election or referendum in place of—
(a)in the case of form 1, the corresponding number list (M1);
(b)in the case of form 2, the corresponding number lists (M2);
(c)in the case of form 3, the form of postal voting statement (for use where there is a joint issue and receipt of postal ballot papers);
(d)in the case of form 4, the form of postal voting statement (for use when a local government poll is combined with another poll but the postal ballot papers are not combined);
(e)in the case of form 5, the official poll card;
(f)in the case of form 6, the official postal poll card;
(g)in the case of form 7, the official proxy poll card;
(h)in the case of form 8, the official proxy postal poll card; and
(i)in case of form 9, form of certificate of employment.
(2) Form 10 is prescribed as the form of words partly in Welsh and partly in English which must be used at a principal area election taken with a relevant election or referendum in place of the words of the notice prescribed by rule 26(8) in Schedule 3 to the 2006 Rules.
