Regulation 3

THE SCHEDULEINFORMATION TO BE INCLUDED IN APPLICATIONS

1.  The date of birth (in the case of an individual or a corporate body only) and the full name of the applicant and any person in charge, and any former names or aliases. Where day care is provided by a partnership, committee or corporate or unincorporate body the dates of birth and full names, and any former names or aliases, of the partners, committee members, directors or governors (identifying any Chairman, Secretary or Treasurer or any person holding a comparable position in the organisation) must also be given.

2.  The address of the premises at which the children are to be looked after and the addresses of the applicant, any person in charge and any other person mentioned in paragraph 1.

3.  Whether the premises at which the children are to be looked after are domestic premises.

4.  In the case of day care, a description of the facilities to be used for the provision of day care on the premises including the number of rooms, their functions, the numbers of lavatories and washbasins, any separate facilities for adult workers and access to the premises for cars.

5.  Whether the applicant wishes to register as a child minder or as a provider of day care.

6.  The proposed hours for which the applicant wishes to provide child minding or day care.

7.  Relevant experience of the applicant and any person in charge including any previous work with children or with elderly or disabled people, whether paid or not.

8.  The number and ages of any children of the applicant and any person in charge and of any children for whom either is to be responsible (other than as a child minder or provider of day care) during the hours in which the child minding or day care is to be provided.

9.  Any relevant qualifications (with dates) of the applicant and any person in charge giving details of the organisation running the course, the subjects studied, the length of the course and the name of the qualification.

10.  Details of the employment history of the applicant and any person in charge including the name and address of any present employer and the names and addresses of any previous employers together with details of any business which the applicant or person in charge carries on or previously carried on. Where there is any period in the employment history of the applicant or person in charge which is unaccounted for an explanation shall be given of the reason for such a period.

11.  The names and addresses of two referees for both the applicant and any person in charge.

12.  The name and address of the general medical practitioner of the applicant and of any person in charge together with details of anything for which the applicant or person in charge is currently being treated by his general medical practitioner or by a hospital and details of any hospital admissions within the period of 2 years, and of any serious illness within the period of 5 years, before the date of the application.

13.  In the case of child minding, the name and date of birth of anyone who is to look after children on the premises where the child minding is, or is likely, to take place and of anyone who is living or employed on such premises.

14.  In the case of day care, the name and date of birth of anyone who is to look after children on the premises to which the application relates (describing the nature of the care that each person will be providing and identifying any deputy to the person in charge) and of anyone who is living or working on those premises (provided that persons should not for the purpose of this paragraph be treated as working on premises if none of their work is done in the part of the premises in which children are looked after or if they do not work on the premises at times when children are looked after there).

15.  The address of each person mentioned in paragraph 13 or 14.

16.  Details of any criminal convictions of the applicant, person in charge and any person mentioned in paragraph 13 or 14 including—

(a)the date of the offence,

(b)the nature of the offence,

(c)the place where the offence occurred,

(d)the name of the court which gave the conviction,

(e)the penalty imposed.

17.  In respect of each person mentioned in paragraph 13 or 14, the name and address of that person’s general medical practitioner together with details of anything for which he is currently being treated by his general medical practitioner or by a hospital and details of any hospital admissions within the period of 2 years, and of any serious illness within the period of 5 years, before the date of the application.
