(2) In the form—
(a)for “European Union (*)” substitute “United Kingdom”;
(b)in the first place the words occur, for “Competent Authority” substitute “Civil Aviation Authority”;
(c)in both places it occurs, for “[competent authority]” substitute “Civil Aviation Authority”;
(d)omit the note at “(*)”;
(e)for “EASA Form 148” substitute “CAA Form 148”.
(3) In the attachment to the aero-medical examiner certificate, for “[Competent Authority]” substitute “CAA”.
