Regulation 4(1)(a)
1. The name and address of the applicant.
2. The name and address of any agent for the applicant.
3. The name and address of the owner of the property.
4. A brief description of the work in respect of which the warrant or amendment to warrant is being sought.
5. A postal address (which in the case of a building must accord with the address for that building in the building standards register) or other description sufficient to identify the subjects.
6. Current use of the building.
7. Information for fee calculation.
8. A declaration by the applicant as to–
(a)the accuracy of the information on the application; and
(b)that the owner of the subjects is aware of the application.
9. Where appropriate, certification in accordance with section 11 of the Act by a certifier of design approved at the date of the certificate in relation to the matters so certified.
