Application for a licence3

An application for a licence shall be in writing in the form set out in Schedule 1 and shall be signed by the applicant and a parent of the child and shall be accompanied by the documents specified therein.

The applicant shall be the person responsible for the production of the performance.

The licensing authority may refuse to grant a licence if the application is not received by it at least 30 days before the day on which the first performance is due to take place.
