Issued in Czechoslovakia from 1.7.1964 to 30.6.1986

Description: Three-part licence; paper pages affixed to flexible plastic folding cover. Pink, cover colour may differ slightly. The paper has a net security pattern. The photograph (35mm × 45mm) is tagged to the frame space on page 1 with an official stamp including the name of the issuing authority and the State coat of arms.
