17Darparu gwybodaeth, cyngor a chynhorthwy

(1)Rhaid i awdurdod lleol sicrhau y darperir gwasanaeth i roi i bobl—

(a)gwybodaeth a chyngor sy’n ymwneud â gofal a chymorth, a

(b)cynhorthwy i gael gafael ar ofal a chymorth.

(2)Yn is-adran (1)(a), mae “gwybodaeth” yn cynnwys gwybodaeth ariannol (gan gynnwys gwybodaeth am daliadau uniongyrchol), ond nid yw’n gyfyngedig i’r wybodaeth honno.

(3)Rhaid i’r awdurdod lleol geisio sicrhau bod y gwasanaeth—

(a)yn ddigonol i alluogi person i wneud cynlluniau ar gyfer diwallu anghenion am ofal a chymorth a allai godi, a

(b)yn darparu gwybodaeth, cyngor a chynhorthwy i berson mewn modd sy’n hygyrch i’r person hwnnw.

(4)Rhaid i’r gwasanaeth gynnwys, o leiaf, gyhoeddi gwybodaeth a chyngor am y materion a ganlyn—

(a)y system y darperir ar ei chyfer gan y Ddeddf hon a’r modd y mae’r system yn gweithredu yn ardal yr awdurdod,

(b)y mathau o ofal a chymorth sydd ar gael yn ardal yr awdurdod,

(c)sut i gael gafael ar y gofal a’r cymorth sydd ar gael, a

(d)sut i leisio pryderon am lesiant person y mae’n ymddangos bod arno anghenion am ofal a chymorth.

(5)Rhaid i Fwrdd Iechyd Lleol neu Ymddiriedolaeth GIG sy’n darparu gwasanaethau mewn ardal awdurdod lleol, at ddibenion yr adran hon, ddarparu gwybodaeth i’r awdurdod lleol hwnnw am y gofal a’r cymorth y mae’n eu darparu yn ardal yr awdurdod lleol.

(6)Caiff dau neu fwy o awdurdodau lleol sicrhau ar y cyd fod gwasanaeth yn cael ei ddarparu o dan yr adran hon i’w hardal gyfun; a phan fônt yn gwneud hynny—

(a)mae cyfeiriadau yn yr adran hon at awdurdod lleol i’w darllen fel cyfeiriadau at yr awdurdodau yn gweithredu ar y cyd, a

(b)mae cyfeiriadau yn yr adran hon at ardal awdurdod lleol i’w darllen fel cyfeiriadau at yr ardal gyfun.

(7)Yn yr adran hon, mae “gofal a chymorth” yn cynnwys cymorth i ofalwyr.
